1. An Unlicensed Assistive Person application
   1. If you have not created your North Dakota Nurse Portal account in the ORBS licensing system, you will need to create one first, then select the UAP application type for your registry type.
   2. If you already have created your North Dakota Nurse Portal account, you will need to log into that account and then select the UAP application type for your registry type.
2. Pay the nonrefundable application fee.
3. A criminal history record check (and fees associated with this) according to chapter 54-07-02.1.
4. Technicians must also submit one of the following:
   1. Verification of completion of a board-recognized formal training program.
      1. Official Transcript must be submitted to the ND Board of Nursing by the school/program if applicable.
   2. Current certification by a board-recognized national body.
      1. Current certification by a board-recognized national body.
         1. AAMA, NHA, NCCT, AMT, NBSTSA, NNCC, BONENT, NAHP or AAH
5. If applying as a student technician or nursing student, you must submit the Verification of Enrollment Form in lieu of #4, available here: SFN 60216 – Verification of Eligibility Form – UAP/Technician
6. If applying as a school personnel, you must submit your medication administration certificate (if applicable) and training checklist signed by the licensed nurse that did the training.

Upon receipt of an application for UAP registration, payment of the application fee, and evidence that the applicant will meet all the requirements for registration in North Dakota, the board may issue a temporary permit to practice in this state. The temporary permit expires at the end of 90 days and is non-renewable. The applicant is responsible for completing all requirements for registry prior to the end of the 90-day permit.
