Unlicensed Assistive Person (UAP) & Medication Assistant III (MAIII) Renewal/Reactivation
Important Update: Transition to New Licensing System
The North Dakota Board of Nursing (NDBON) is excited to announce that our new licensing system is now live. You can access the system by clicking “Nurse Portal” in the above navigation bar or by clicking here.
Please note in order to renew your UAP/MAIII registration, you will be required to create an account in the Nurse Portal. For additional information, including how to setup your account, click here.
All individuals listed under the UAP/MAIII registry must renew their registration every two years. All registrations expire on June 30th of the year in which they expire. The renewal period opens April 1st and runs through 11:59 p.m. June 30th. All registrations not renewed in that time frame will expire and practice is not allowed.
You must complete the renewal application inside the Nurse Portal and attest to continued competency as outlined below:
UAP Renewal Requirements
School Personnel:
- Current Employment, or Past Employment within the past two years
- You are considered employed as a UAP if you are holding a UAP School Personnel registration for purposes of your job within the school/district.
Surgical/Dialysis Technicians and Medical Assistants:
- Current Employment, or Past Employment within the past two years
OR
- Active national certification with an expiration date beyond June 30th of the current year from one of the below corresponding certifying agencies:

- If you do not meet one of the above requirements, and your certification expires on or prior to June 30th of the current year, you are required to renew it with your certifying body prior to completing this renewal.
Student Technician (Surgical/Dialysis/Medical Assistant):
- Current enrollment in a technician program as verified by your program. Enrollment must be verified by having your technician program download and submit back to the NDBON the following form: SFN 60216 – Verification of Enrollment – UAP/Technician
- If you have completed/graduated from your program, have your program send to us an official copy of your transcript with your degree and/or completion posted to the ND BON to update your status to a full, non-student technician status before completing your renewal application.
Nursing Student:
- Current enrollment in a nursing program as verified by your program. Enrollment must be verified by having your nursing program download and submit back to the NDBON the following form: SFN 60216 – Verification of Enrollment – UAP/Technician.
MAIII Renewal Requirements
Medical Assistant
- Active certification with an expiration date beyond June 30th of the current year from one of the below certifying bodies:

- If your certification expires on or prior to June 30th of the current year, you are required to renew it with your certifying body prior to completing this renewal.
- Note you must also renew your UAP registration in addition to the MAIII.
Nursing Student
- Completion of the following form by your nursing program: SFN 53337 – Verification of Eligibility for Medication Assistant
- Note you must also renew your UAP registration in addition to the MAIII.
Late Renewal (July 1st through July 31st)
From July 1st through 11:59 p.m. July 31st, registrations that expired in the current year can renew as a “late renewal” with an additional cost of double the timely renewal fee. MAIII registrations will also need to renew their UAP and MAIII registrations separately, as opposed to completing a combined renewal if done during the normal renewal period. Verification of practice may be required if you renew after your registration has expired.
Reactivation
All registrations still expired on June 30th of the current year that are not renewed by the close of the late renewal period on July 31st 11:59 p.m. will require reactivation (including completion of a Criminal History Records Check) to become active again.
