Application for Approval of Contact Hours
for Continuing Education
Individuals, organizations or agencies desiring approval of contact hours for a continuing education activity or event must complete and submit the Application for Approval of Contact Hours, supporting materials, and fee payment.
Please review the following instructions:
- A completed Application for Approval of Contact Hours (SFN 54001) and fee payment are to be submitted thirty (30) days prior to the first offering date.
- The fee must accompany the application and supporting materials if mailed or be paid when the application is submitted electronically.
Application Fees:
- $200 Application Fee (Includes one contact hour)
- $40 for each additional contact hour being requested (Example: Three contact hours requested- $200 + $80 = $280 fee)
- Fees are not prorated for less than one contact hour. If requesting a portion of a contact hour, round up to the nearest whole. Example: Fee for 1.5 contact hours = $240 (calculate for 2.0)
- Please allow 15 business days for Continuing Education contact hour approval. Approval is valid for one year for all approved activities.
